Week 2:Comprehensive Psychiatric Evaluation Note and Patient Case Presentation

Subjective CC (chief complaint)

The patient is a 25-year-old woman has stopped taking her mediation since she claims they are not working. Her chief complaint is, "my mother called the police on me since she wanted me to go back to my boyfriend.” HPI P.K is a 25-year-old African woman who presents signs of mood disorder. She has stopped taking her medications simply because they do not make her feel good.Additionally, she is an active rug user who has had an overdose and suicide case before. Her behavior forced her mum to call the police on her. It was after evaluation that she was recommended to visit a psychiatrist. She complains that she hears voices when she cannot sleep at night. Similarly, her sleeping pattern is irregular depending on her moods. Past Psychiatric History General Statement:The patient has previously been diagnosed with depression. Caregivers (if applicable):Not applicable. Hospitalizations: The patient has been hospitalized four times. One of the time was due to a suicide attempt, while another was due to an overdose of Bendery.
