S.O.A.P Notes: A Patient Present in the Clinic with Left Flank Pain

 

Subjective data: Patient’s Chief Complaint

PS is a 39 y/o woman who complains of progressive, severe left flank and lower back

pain for the past three days. She describes the pain as sharp as she could barely sit comfortably

on her couch. She requires a wheelchair to move around the house and is concerned that her

condition is becoming worse.

She was diagnosed with muscular low-back strain. She has been taking anti-inflammatory

medications that are non-steroidal. She was also using oral narcotics to help with the left flank

pain. She also has a prominent left ovarian cyst, which was thought to be the source of the pain.
