Project Summary
The project main aim is to evaluate the impact of long-term care on nursing practice, and how they impact patient outcomes in healthcare environment. Various approaches that can help improve patient care are recommended. The preferred methodology and how they will be implemented to meet project goals are evaluated. Reflection on Objectives The objectives are necessary to explore the topic of study in the field of nursing and overall healthcare. The impact of a particular topic of study need evaluation on the long-term care management practices that affects the patients and staff related issues within the healthcare facility. The main objectives that will be considered are concerned with evaluating the impact of care management on nursing practices in the long term, and how this engagement impact patient in the healthcare setting and the practices of the nursing personnel within a professional care environment. Besides, the project will be concerned with recommending the various approaches that can be utilized while establishing long term care management for patients and improve issues related to patient care within healthcare environment. In addition it will be concerned with exploring various processes and factors that should be considered while developing long term care management plan, which is aimed at the promotion and optimization of positive outcomes among the members of staff that operate within this environment. Further, the other goal of the practicum project would be to investigate the implication of the long term care management plan on nursing practice on different issues that affect the nursing staff and in the nursing care environment (Garner & Wilson, 2019)
