 Hello all, my name is Michelle Chavis and I will present today the important issue of care coordination in interdisciplinary settings which remains a vital component of healthcare effectiveness and sustainability in the modern era of technological development.  Solutions and facilities, which is care coordination. This presentation will address all the nursing staff and related stakeholders and address the concept of care coordination that plays a vital role in the improvement of care quality of treatment. Care coordination is believed to associated with several positive outcomes and results such as improving the affordability of healthcare services in America. Patients, healthcare professionals, and family members will all benefit from my presentation. Recently, there has been a surge in the patients commonly suffering from chronic diseases due to COVID-19 and other habitual issues. The benefit of this presentation is for all those patients having complicated medical conditions including hyperglycemia that is associated with blood glucose irregularity and causes diabetes and stress in people. 

Evidence-Based Effective Strategies for Collaborating with Patients and Their Families 

Giving appropriate treatment to these patients is important through team and professional collaboration to achieve efficient outcomes. Many healthcare experts in our organization believe that interprofessional collaboration can help us to reduce the events of misdiagnosis. The important of coloration and continuum of care is increasing day by day; these are some vital components of a good healthcare culture that does not happen by itself. For example, among nurses and physicians, communication is described by the Agency for Healthcare Research and Quality (AHRQ) as a conscious and deliberate effort to improve the acceptability and availability of healthcare quality services for the patients (Young, 2020). Collaboration is deeply associated with healthcare administration and the incorporation of the cutting-edge technology such as electronic health records EHR systems Nurses can enhance the level of coordination by communicating their healthcare expertise with other employees including senior and junior coworkers to form a knowledgeable team. They can strive to ensure smooth healthcare delivery and service across all wards and emergency departments.  Collaboration among several physicians including finding a tailored approach that is able to provide an adaptive treatment to meet the needs of patients. Coordination can empower patients in achieving the quality treatment that is lacking in other organizations not following the guidelines of the American Nurses Association (Young, 2020). For example, according to the guidelines of the ANA, nurses should not allow excessive vests per day per patient in the hospital to avoid any quality issues. The self-management of patients should be encouraged through raising their confidence in treatment. Moreover, the healthcare expenses should also be reduced to increase the rate of survivors not financially sought-after. This means that good coordination allows nurses to get familiarized with national frameworks to improve their skills.  

Culturally Sensitive Strategies for Collaborating with Patients

In this presentation, the culturally sensitive communication techniques are to be implemented in different mechanisms. I will consider my own sociological aspects in the light of cultural frameworks and will analyze norms to improve the honest collaboration between stakeholders. The role of empathetic collaboration is pivotal in healthcare organizations employing diverse employees. The cultural differences are obvious and evident in people who think differently with a range of ideas, behaviors, and traditions (Wittenberg, 2019). For example, nursing staff and practitioners must be able to understand specific health interactions to understand the viewpoint of patients and their families. The assessment of how customers can effectively communicate in a culturally sensitive manner. Every individual has a different attitude based on their beliefs and their culture; which is why effective communication and appreciation for an individual’s traditional attitudes is indispensable to help them engage in honest collaboration. This includes communicating with patients and coworkers clearly by using effective listening techniques and use compassion that helps to create a bond between stakeholders.
