CC (chief complaint): “I have had worsening cases of an alternation of periods of high and lows”
HPI: The patient ML is a 24-year-old woman. She is living with her parents and his grandparents. The patient has a history of generalized anxiety disorder and major depressive disorder. The patient is taking medication for her anxiety. She is using Lexapro 10mg a day and hydroxyzine 5- mg S. She has stated that she has a challenge of feeling high for a period of up to 10 days where she experiences issues of racing thoughts and that she talks very fast. The other symptoms that are presented by the patient are cases of having impulsive shopping, insomnia, talking fast, and changing topics. She has presented that she was hypersexual in the past when she had one of her episodes where she had more than one partner over a few days that she had the urge.
She states that during the occurrence of the episodes she is unable to concentrate. She states that the racing thoughts are a bother to get since she cannot be able to cope with them. She intimates that the high episodes alternate with the low episodes but she has the feeling that she does not want to show that there are people who are helping her. She has denied having any other medical condition from the past that has led to her hospitalization. She reports that she does not have any known allergies and does not make use of any drugs nor does she drink alcohol. The family of the patient is living with her parents and paternal grandparents. She has stated that her parents are healthy and that her grandparents are in their 80s and are suffering from diabetes and hypertension…
