Subjective: CC(chief complaint): “I was living and not bothering anyone, and those people..those people, they just won’t leave me alone” HPI: A 53-year-old man named S.T., of Dominican descent and raised in the United States, visited the clinic accompanied by his sister. He reported that he has been followed by strangers and his personal space has been invaded, causing him significant distress. He mentioned living with his mother until her passing three years ago, after which he started perceiving the presence of spies watching him. He reported to be able to see their shadows using glass and hear their conversations, which has been happening over a couple of weeks. According to the patient, these individuals are government agents assigned to watch him, leading him to believe that his taxes are unreasonably high. He also described hearing heavy metal music that is very loud and seeing a bird in the room, although these observations were not evident during the assessment. He believes that the spies are trying to convince him that the music is not loud, but he insists that it disrupts his sleep and keeps him awake for extended periods. He mentioned that he usually watches television when he has problems sleep, but he believes the spies are using the TV to monitor him. He further claimed that the spies have broken into his home and contaminated his food, but he has managed to outsmart them by hiding everything in the refrigerator, where they cannot reach. Due to the ongoing surveillance, he avoids going to the grocery store and depends on his sister for shopping.
