Subjective:

CC (chief complaint): ”  I have a history of taking medications and then stopping them. I don’t think I need them. I feel like the medication squashes who I am”

HPI: The patient is a 27-year-old woman called Petunia Park. She is presented to the clinic with the complaint of being unable to take medication during some instances due to the way that they make her feel. The patient as well reports that there are 4 to 5 times in a year that she feels that she does not want to get out of bed, does not have energy, lacks motivation, and cannot have any interest in her creativity. This is the time that she does not take her medication as it makes her squash but every time she ends up being crashed. She has had more than 4 hospitalizations including the issue of trying to commit suicide and one time that she states that she was picked up by the police and taken to hospital when she was in the park playing the guitar without clothes. She however cannot remember this. She indicates that his brother is seen to be “crazy” and some say that he has bipolar. His mother has tried suicide. His father was jailed due to the use of drugs. She admits heading things but this was months ago that talked to her hence she heard voices. She has denied having any lack of appetite unless when she is having her high moments that she finds to be creative and cannot remember to take food. She has enough sleep but during the episodes, she gets 3-4 hours of sleep. She sleeps for 12-16 hours when she crashes. She has admitted to taking nicotine and that she cannot stop. She does not take alcohol, she indicates that she took alcohol last time when she was 19 years old….
