S –  The patient is has been brought to the health setting with the complaint of having shortness of breath, and on having an examination, it is found that he is suffering from heart failure exacerbation. The review of the patient shows that he has an elevated level of BNP. This shows that the patient has a high level of a substance released by the ventricles for the improvement of the circulatory homeostasis since it helps in acting on the blood vessel. The other thing is that it is found that the patient has pulmonary edema. This is a condition that may be found from carrying out the imaging of the patient. The patient as well can get a chest x-ray to help in confirming the diagnosis. The issue of pulmonary edema is the main cause of the shortness of breath that is related to heart failure exacerbation.  The patient needs help to be able to have an improvement in his health condition and has been asked 120 unique questions on his health. Out of the 120 questions that the patient is asked, 45 key questions are asked to help in the process of determination of the diagnosis of the patient.
