Analysis of the scenario shows that the state of the patient is different from what is indicated in charting. In this case, the physician breached the ethical principle of veracity which emphasizes integrity and honesty. From an interprofessional perspective, this kind of a situation necessitates adopting an approach in which one engages the physician, sharing with them the problem identified during the patient rounds. While discussing the problem, it is important to explain why it is important to record correct details about the patient’s state.

Focusing on collaboration as a strategy for mitigating such an issue is equally crucial because through teamwork, it is possible to manage issues that contribute to the disorientation of the patient (Varkey, 2021). By adopting collaborative measures, for instance, they can recommend making changes to the type of intervention used or suggesting assessment of the patient. In responding to the scenario, one may have to focus on advocating for the welfare of the patient by requiring providers to provide accurate details of the patient’s physical health status (Varkey, 2021).

The culture that an organization adopts can determine how effective it is able to manage errors.  A culture in which leaders express commitment to safety, for instance, can lead to a situation in which they mobilize the clinical staff to deliver error-free health services. Banking on a solid safety culture, an organization may regularly audit its clinical procedures, and take appropriate measures to rectify any issues that may compromise the quality of clinical services (Cao et al., 2023). Furthermore, this kind of culture may motivate employees to be transparent and honest, which ultimately reduces the chances of occurrence of a similar issue.
