Introduction

The understanding of the extensive learning measures has been seen in the past 5 weeks with the development of my skills being evident each week. The progressive development of the skills and practice knowledge has been obtained over the past 5 weeks. The clinical logs will help in the reflection of the success obtained during the five weeks with each week has been an instrumental learning process. The skills gained during each of the weeks will be applied in the process of provision of care. The topic of interest that has been selected is the fall of elderly patients in long-term nursing homes.

Week 1: Introduction to the second clinical practicum

During this week there was an introduction to the clinical practicum topic. The main idea is on the selection of the practicum site. Specifically, the practicum site is the area of concern that is expected to help in the creation of a learning instance and that allow for the development of the basis of change in the community. The prospect of understanding the area of interest is found to be instrumental as it allows for the setting up of a process to ensure that there is patient-centered care being offered.

The experience gained in the first clinical practicum helped in the provision of a basis for handling this practicum procedure to handle the cases of synthesis of knowledge being seen as an experience for the creation of the concurrent working relations and that allows for the understanding of the essentials of care that is supposed to be always adhered to in the process of provision of patient care.
