Capstone Change Project Evaluation Plan

 

The proposed change is reduction of cases of central line associated bloodstream infections (CLABSIs) through adoption of evidence-based practices. The objective of the capstone is to train nurses and administer evidence-based practice in order to overcome the reduce the case of CLABSIs based on evidence-based practices. The EBP proposed for this project include handwashing, use of chlorohexidine regular to clean catheter site injection and removal of all catheter considered unnecessary (Anjomshoae, Hassan, Kunz, Wong & de Leeuw, 2017). These practices require resources in order to materialized. hence the required resources are nurse educationist, management support, nurses as human resources and supplies such as hand sanitizers and chlorohexidine. The process of evaluating the intervention when implemented is a survey tool and score card. The use of survey is vital since it helps in tracing inputs and comparing with outcomes. Also, the survey allows for comparison of actual outcome as compared to anticipated or project outcomes
