Milestone One Intervention An evidence-based program for heart disease and stroke is the intervention. This program is called Interventions Engaging Community Health Workers. This program helps identify healthcare professionals who are involved in the public health sector who are either from the local area or understand the area and the needs of the community in which they are servicing. Interventions Engaging Community Health Workers aim to help prevent both heart disease and stokes by providing specialized services including education, counseling, social support, as well as additional recourses people in the community may need to provide healthcare services through various models of care (Healthy People, 2015). Evidence also shows that the Interventions Engaging Community Health Workers who are members of clinical support teams are able to provide an even greater amount of assistance to those who may encounter various obstacles when trying to obtain care in an underserved area in order to help decrease health disparities (Evidence of Impact for Interventions Engaging Community Health Workers, 2022) Prevention Level Interventions Engaging Community Health Workers operates within the primary prevention level. It aims to intervene in care plans prior to any negative or adverse outcomes can occur (CDC, 2022). Unfortunately, in some cases, if a patient has already developed a form of heart disease, secondary prevention level would be the next step. During this step, the heart disease can hopefully be caught in the early stages in order to prevent and/or reduce any further damage. Both heart disease and stroke prevention methods are imperative to be performed prior to any serious, permanent damage is done to one's heart and body that would be unable to be reversed. Interventions Engaging Community Health Workers goal is to be an advocate for the community in which they serve to ensure individuals are informed and obtain the proper care to prevent any future chances of developing heart disease or strokes. Interventions Engaging Community Health Workers has shown results which were effective both internally and externally which enables them to reach their goals (Evidence of Impact for Interventions Engaging Community Health Workers, 2022). Health Service Delivered Having both professional and nonprofessional healthcare workers focus solely on cardiovascular disease prevention is the main public health service that is delivered. This prevention can lead to heart disease and strokes being prevented by providing education such as well-being teachings, evaluations, outreach, patient steering, and organization liaisons (Healthy People, 2015). Interventions Engaging Community Health Workers is comprised of various healthcare representatives, advisors, and other professionals who are within the category of "front-line public workers" who have the ability to aid in the mending if the community within the healthcare systems. Interventions Engaging Community Health Workers clinical team is also able to aid in the improvement of knowledge, medication, awareness, and participation in care of the community in which they serve (Evidence of Impact for Interventions Engaging Community Health Workers, 2022)
