What are the different kinds of nursing care plans?

There are two types of care plans available: formal and informal. A formal nursing care plan is a written, standardised care plan that is created after organizing information about the patient's condition. An informal nursing care plan exists only in the nurses' minds. For example, in a formal nursing care plan, if a patient has a fever, the nurse must plan to lower the temperature. A nurse's responsibilities may include cold sponging, providing a calm and quiet environment, checking the temperature at regular intervals, and so on. Informal nursing care plan: If a nurse notices that a patient's hair is unkempt, he or she will think to comb the hair properly. It only exists in the nurse's mind and does not necessitate any proper planning. Aside from this, there are two types of care plans available: actual and psychosocial. The patient's problem is the actual care plan. If the patient has diarrhoea, the pain will be covered by the actual care plan. Because of the actual problem as well as what other problems the patient is experiencing. If the patient has one hand amputated, he may lack self-confidence and believe that he will be dependent on other family members. A psychosocial care plan is used to address this type of issue. The nursing care plan's goal Every activity performed by nurses must be purposeful and goal-oriented. The following are the goals of the care plan: It provides nurses with the necessary course of action for the patient. A care plan serves as a means of communication among members of the health care team. It aids in the identification of the patient's various problems. A care plan allows nurses to get to know their patients better by encouraging critical thinking about patient problems. A record of the care plan ensures continuity of care even if nursing personnel change shifts.
