Explanation of SDOH Impact on Subpopulation According to County Health Rankings, the premature deaths rank 7,200 per 100,000 people in Coles County where as Illinois is at 7,100 per 100,000 people and the United States is at 7,300 per 100,000 people. Additionally, average length of life in Coles County is 77.7 years old, which is lower than that of Illinois at 78.6 years and the United States at 78.5 years. Heart disease was the most common cause of dealth in Coles County. Obestity and lack of exercise can lead to heart disease. 36% of adults are obese in Coles County, only 64% of the county have access to exercise opportunities, versus the 87% of the population for the state. In coles county 27% of the population has no physical activity. These can contribute to adult obesity, which is increasing according to the Center for Disease Control and Prevention (2021). Obesity and poor management of chronic conditions can lead to premature death. Sarah Bush Lincoln is the hospital in Coles County. I reviewed the hospital website and reviewed their Community needs assessment that they performed in 2021. According to the assessment poper food access is a main limitation in Coles County. “This indicator reports the percentage of the population living in census tracts designated as food deserts. A food desert is defined as a low-income census tract where a substantial number or share of residents has low access to a supermarket or large grocery stores. The information is relevant because it highlights populations facing food insecurity. 28.97% of Coles County have low food access compared to 9.69% in Illinois.” (Sarah Bush Lincoln 2021)
