Background Diabetes is a growing health concern and will continue to affect millions of middle-aged adults for the next few decades unless action occurs. Diabetes can cause patients to have severe complications in their overall health and worsen any preexisting conditions. The most significant complications that diabetes is responsible for are hypoglycemic/hyperglycemic events, difficulty in controlling hemoglobin A1C levels, and medication management. The progression of diabetes is capable and not as difficult as patients may seem. The goal of this Capstone change project proposal is to target the most prevalent population that is being affected by diabetes and inspire a systemic change that can change the trajected course of diabetes growth in middle-aged adults. The most effective practice that healthcare was founded on is consistent education and retraining. Many professions in the medical field require healthcare professionals to stay familiar and proficient with the most current treatment and management of all diseases. If patients are assisted by healthcare professionals by partaking in this patient education initiative, then the confidence of patients to treat, manage, and prevent diabetes would improve patients' overall health. Clinical Problem Statement Diabetes type 2 is a chronic health issue that is one of the most globally impacting diseases. Diabetes is responsible for a large amount of financial burden in healthcare and health insurance. "The International Diabetes Federation estimated that 12% of global health expenditure is spent on diabetes and, despite the advances in both modern medicine and knowledge of the disease, it remains one of the leading causes of death globally" (Raimundo et al., 2020). However, diabetes type 2 is one of the most easily preventable diseases. The capstone project change proposal is to assist the prevention and treatment by re-educating patients of the essential knowledge and advice to reduce the progression of diabetes
